As we begin planning for the 2015 meeting I want to take a moment to thank the meeting planners and ALL who helped make 2014 come together.. Dr.. Gretchen Dickson chaired the Professional Development committee.. Moderators Drs.. Eric Clarkson, John Feehan, Sheryl Beard, Chad Johanning and Jeremy Presley were great.. Other major forces in the implementation were KAFP-Foundation Board members who assisted in setting up and working the Silent Auction, Dr.. John and Deb Feehan, who welcomed us into their home for the Wine Tasting fundraiser, and Dr.. Jen Brull who was involved in almost everything, and the 4th year students who helped her run the Latex to Laryngoscopy student track on Saturday morning..     Descriptive info: President's Corner.. by.. Mary Beth Miller, MD.. The Future is NOW: PCMH.. As we all know, changes to our healthcare system are coming.. What we don t know is what the new system will look like.. And if we could imagine what it will be after all the changes take place as we develop this new model.. I often wonder if Walt Disney ever knew that his Disneyworld would transform into a gigantic entertainment place from his simple model he had for Disneyland.. The best understood part of healthcare change has been going on for over a decade with the development of the PCMH (patient centered medical home).. For most of us in family medicine, this is what we have always believed we have had.. And that is a fine goal to profess.. In reality, what we have achieved has probably been more patient centered in our minds than in reality.. Right now, there is a mix of opinion across our state and among our colleagues about what PCMH is and its value.. Some practices have already achieved the NCQA recognition for being a PCMH model, others are working on it, others aren t exactly sure what any of those letters mean, and still others are adamant that this is just another fad.. PCMH is the model of clinical care that provides the framework needed to make sure what we do in reality is truly patient centered.. This is achieved and described by the Seven Joint Principles of the PCMH:.. Personal physician: each patient will have an ongoing relationship with a personal physician who will provide first contact and continuous comprehensive care.. 2.. Physician directed medical practice: the personal physician directs a team of individuals who are responsible for the ongoing care of each patient.. Each member of this team is expected to work to the top of their license.. 3.. Whole person orientation: the personal physician provides for and coordinates care with other qualified professionals through all stages of care: acute, chronic, preventive, and end of life.. 4.. Care is coordinated/integrated: care is coordinated across all elements including specialty care, hospitalization, home health, long term care, and the services available in the community.. This coordination is then followed by appropriate follow up of the patient through this chain of care.. 5.. Enhanced access to care: Making access better through such concepts as open scheduling, expanded hours, and allowing for new options of electronic communication with patients such as email and portal access to the medical record.. Safety and quality: focus on evidence based medical practice guideline, chronic disease management with the help of registries and clinical decision making tools, appropriate use of information technology, and ongoing quality measurement and continuous quality improvement.. 7.. Payment systems to appropriately recognize the added value of a PCMH.. NCQA is  ...   team in your office that represents all areas of the practice from reception, to billing, to nursing, medical assistants, mid-level providers, and a physician.. You want this team to be made up of people in the office that tend to be progressive and not change averse.. The team needs to understand that the process is not one of weeks, but rather of years.. It is also going to be an ongoing initiative as you adjust your system to make it better over time.. I recommending starting with a tool that lets you see where you are now in your practice in terms of the NCQA standards.. There is a tool on their website at.. ncqa.. org/.. I.. f possible, I would have the entire staff fill out the questionnaire to get a complete perspective.. Does everyone see care being provided in the same way?.. Often it is the physician who least understands the hurdles our patients often have in dealing with our office.. The next most important step will be setting aside the time it takes to have regular meetings with the team and the rest of the office as you go through the process.. Don t underestimate the importance of regular meetings.. It keeps everyone informed and keeps the process moving forward.. Try to set goals at the end of every meeting along with a time frame as this also keeps the work moving.. There are all sorts of tools, companies, websites, etc to help you in your transformation.. Don t be afraid to access these.. Easily accessible is the Kansas PCMH Coordinator, Lesla Roach.. Feel free to call her 316-721-9005 to give you some pointers.. Planning ahead can sometimes help you avoid the more difficult problems that will arise.. Be in the project for the long run.. In addition, AAFP is getting ready to roll out a PCMH Planner early in 2014 that promises to be a great way to move forward in this important effort.. Stay tuned to hear more about it!.. On a personal note, I enjoy long distance running.. One of my favorite posters shows a solitary runner out on a lonely road running.. The underlying quotation: the race is not always to the swift, but to the one who keeps on going.. I think this fits nearly any change we face in life.. Whether that be developing EMR in our office or becoming the practice of the future fully invested in patient centered medical care.. Are YOU ready to start? Are you ready to keep going if you are already engaged in the transformation? Are you ready to embrace continued improvements if you have already achieved Level 3 NCQA recognition? Wherever you are, you are not alone.. And it will be well worth it if you keep on going.. Sincerely.. KAFP President..

Cash awards are given to students, residents and fellows in family medicine who present their research findings through a poster session at the KAFP Annual Meeting Scientific Session.. Download the 2012-13 STUDENT RESIDENT POSTER CONTEST APPLICATION.. Rural Family Medicine Practice Research Program.. Students between their first and second year of medical school are sent to rural areas across the state to learn about family medicine from local primary care doctors, and gather research data.. Each year between 25- 35 medical students participate in the program.. This program exposes first year students to the life of a rural family physician.. Thank you to the volunteer faculty and directors of this program for making it happen: Dr.. Mike Kennedy.. Family Medicine Interest Group grants.. Grant.. funding is provided to help these student organizations with their activities.. Creating increased student interest in family medicine, and assisting these student groups is a high priority for the Foundation.. AAFP's National Conference for Medical Students and Residents (NCFMSR) in Kansas City.. This is an opportunity for students to learn about the specialty and the conduct of the political congress.. Scholarships.. to attend this conference are available.. The KAFP-F Hostetter Award.. This award honors a senior medical student on  ...   collaboratives was supported to help Kansas Family Physicians as they learn about and grow toward becoming a PCMH..
